Trails to Success Residential Treatment Program


Trails to Success Residential Philosophy is an individualized, strength based program developed to exceed best practices using a positive approach. Valle Vista Health System exists to be a positive life changing resource to our patients, their families, and our communities.


Assessment

  • CANS (Child and Adolescent Needs and Strengths) assesses life domain functions, strengths acculturation, caregiver strengths and needs, child behavioral /emotional needs, child risk behaviors, and includes a school module, substance use module, violence module, developmental needs module, and a family module.
  • Nursing Assessment reviews current level of physical functioning and activities of daily living, assesses presence of pain, neuro history, and fall risk.
  • Psychiatric Assessment Includes history, current level of functioning, mental status exam, initial treatment plan, recommendation for treatment and criteria for discharge.
  • H & P (History and Physical) Addresses any current illnesses, family medical history, and assesses need for medical treatment required during a residential stay.
  • Biopsychosocial Assessment determines risk factors, substance abuse history and current usage, impaired functioning, losses and/or stressors, support system, family functioning, history of trauma/abuse, legal and employment history, military, spiritual, cultural, sexual and recreational history.
  • Activity Therapy Assessment determines a patient’s area of recreational and leisure interest.
  • Master Problem List of the initial treatment plan developed within 24 hours of admission.

Communication

  • Reports are monthly updates sent to all FCMs, POs, and Dawn Project CMs. Reports provide an overall level of functioning; progress towards treatment goals, medical, school, family. (needs a space on my designer copy )In the event of an incident, notification will be made by telephone and followed by an e-mail.
  • Case Conferences scheduled with all case managers and probation officers needed.

Clinical Interventions

  • Discharge Planning begins the day the patient is admitted. Patients (and family) needs will be identified and situational supports are put into place before the patient returns to the community.
  • Strength Based Individualized Treatment Plan is developed with input from the patient/and family. The treatment team identifies discipline specific interventions to assist patient in attaining an identified goal.
  • Individual therapy at least one time weekly.
  • Group therapy several times daily.
  • Recreational therapy facilitated by a certified recreational therapist.
  • Family therapy occurs at least two times monthly.

Medical Services

  • 24 hour RN services to provide medical over site and management of the medically compromised patient. RNs provide education regarding symptom management and medication.
  • MD available for medical consults.
  • Dental services available on site.
  • Pregnant patients receive prenatal care through Wishard. Pregnant patients receive prenatal and childcare education.
  • Special medical consults are arranged as needed and transport is provided.

Diagnostic and evaluation

  • Psychological testing is a contractual service that is arranged by staff.
  • Personality Inventory for Children (PIY) The PIY assesses emotional and behavioral adjustment, family interaction, and neuro-cognitive and attention-related academic functioning.
